Once purchased, you will receive a document to fill out all of your info and include any important information you need me to know.

Can I change the layout or remove a line?
The layout cannot be altered as this is a pre-designed template. However, if you want the wording to change, (example: you don’t want to include parents’ names and wish to use “Tother with our families”) we can most definitely make that change. Or if there is content in the template you don’t like, we can remove it. Just let me know in the notes section of the form.
